Kanser tanısı ile tedavi ve takip altında olan hastalarda nutrisyonel risk değerlendirmesi tarama testi nutriscore'un Türkçe güvenilirlik ve geçerliliğinin belirlenmesi ve farklı tarama testleri ile karşılaştırılması

Abstract

Kanser hastalarının malnütrisyon riskini belirlemek amacı ile geliştirilen tarama ve değerlendirme araçları, riskin erken dönemde tespiti ve tedavisi için oldukça önemlidir. Bu çalışma; onkoloji polikliniklerinde kanser tanısı ile tedavi ve takip altında olan hastaların malnütrisyon riskini saptamak amacı için tasarlanmış NUTRISCORE tarama testinin Türkçe güvenilirlik ve geçerliliğinin test edilmesi ve farklı tarama testleri ile karşılaştırılarak hastaların beslenme durumlarının belirlenmesi amacı ile metodolojik olarak gerçekleştirilmiştir. Çalışma, Haziran-Ekim 2018 tarihleri arasında Marmara Üniversitesi Pendik Eğitim Araştırma Hastanesi Radyasyon Onkolojisi ve Tıbbi Onkoloji tedavi ünitelerinde aktif tedavi gören 94'ü kadın (%39,2) ve 146'sı erkek (%60,8) olmak üzere toplam 240 yetişkin (?18 yaş) kanser tanılı hasta ile yürütülmüştür. Çalışmada veri toplama araçları olarak; hasta bilgi formu, European Diagnostic Criteria (EDC) tanı kriterleri, NUTRISCORE, Nütrisyon Risk Taraması-2002 (NRS-2002) ve Malnütrisyon Tarama Aracı (MST) tarama testleri kullanılmıştır. NUTRISCORE tarama aracının dil uyarlama çalışmasında, en çok tercih edilen yöntem olan çeviri-geri çeviri yöntemi kullanılmıştır. Kapsam geçerliği için DAVİS tekniği kullanılmış ve 13 uzmanın görüşüne başvurulmuştur. Yapılan hesaplamalarda ölçekteki her maddenin Kapsam Geçerlilik Oranının (KGO) 0,7'den büyük olduğu görülmüş ve toplam Kapsam Geçerlilik İndeksi (KGİ) 0,94 olarak bulunmuştur. Ölçeğin Türkçe formunun güvenilirliğinin değerlendirilmesi amacıyla; test tekrar test yöntemleri kullanılmış ve NRS-2002, MST ve EDC ile arasındaki korelasyona bakılmıştır. Test-tekrar test güvenilirliği için ölçeğin Türkçe formu üç hafta ara ile 67 hastaya uygulanmış ve iki ölçüm sonucu arasında pozitif yönlü, yüksek düzeyde ve istatistiksel olarak önemli bir ilişki bulunmuştur (r=0,971 ; p<0,01). Referans test NRS-2002'ye göre NUTRISCORE, MST ve EDC'nin duyarlılık, özgüllük ve prediktif değerleri hesaplanmış ve ROC eğri grafikleri oluşturulmuştur. Tarama teslerinin arasındaki uyumu belirlemek için Cohen'in kappa katsayısı kullanmıştır. Referans test NRS-2002 ile karşılaştırıldığında NUTRISCORE, MST ve EDC'nin özgüllük değerleri sırası ile %100, %83 ve %91; duyarlılık değerleri ise sırası ile %85, %91 ve %81 olarak hesaplanmıştır. NUTRISCORE, MST ve EDC için AUC değerleri sırası ile %0.96, %0.92 ve % 0.86'dır. Cohen'in kappa istatistiğine göre yapılan hesaplamada NRS-2002 ve NUTRISCORE arası kappa uyumu 0,88; NRS-2002 ve MST arası kappa uyumu 0,34; NRS-2002 ve EDC için 0,73 olarak bulunmuştur. Çalışmamızda Türkçeye valide edilerek incelenen ve yeni bir tarama testi olan NUTRISCORE'un onkoloji polikliniklerinde tanı alan ve tedavi gören kanser hastalarının malnütrisyon riskini yüksek doğrulukta tespit edebildiği ve tarama testi olarak kullanılabileceği sonucuna ulaşılmıştır. This research; NUTRISCORE screening test designed to determine the risk of malnutrition of patients undergoing cancer diagnosis and follow-up in oncology outpatient clinics was performed methodologically in order to test the reliability and validity of Turkish NUTRISCORE screening test and to compare nutritional status of patients with different screening tests. The study was conducted between June-October 2018 in the departments of Radiation Oncology and Medical Oncology outpatient clinics of Marmara University Pendik Training and Research Hospital. A total of 240 patients (%94.3) of whom were female (%39.2) and 146 were male (%60.8) participated in the study voluntarily. In the study, as data collection tools; patient information form, NUTRISCORE, NRS-2002 and Malnutrition Screening Tool (MST) screening tests were used. The language validity of the Turkish form of the scale was provided by the translation-back translation which was the most preferred method. In addition, thirteen experts were consulted for scale content validity and total Content Validity Index (CVI) was found to be 0,94. In order to evaluate the reliability of the Turkish version of the scale; Phi (?) correlation coefficient with NRS-2002, MST and EDC screening tests were examined and test-retest method was used. For test-retest reliability, the Turkish version of the scale was administered to 67 patients with four-week intervals and a positive, high-level and statistically significant relationship was found between the two measurements (r=0.971; p<0.01). The sensitivity, specificity and predictive values of NUTRISCORE, MST and EDC screening tests were calculated according to the reference test NRS-2002 and ROC curve graphs of the screening tests were calculated. Cohen's kappa coefficient was used to determine the fit between the screening tests. Compared to the reference test NRS-2002, the specificity values of NUTRISCORE, MST and EDC screening tests were found to be %100, %83 and %91 respectively. According to the reference test NRS-2002, the sensitivity values of NUTRISCORE, MST and EDC screening tests were calculated as %85, %91 and %81, respectively. The AUC values for NUTRISCORE, MST and EDC are 0.96%, 0.92% and 0.86%, respectively. According to Cohen's kappa statistics, kappa agreement between NRS-2002 and NUTRISCORE was 0,88; kappa agreement between NRS-2002 and MST was 0,34; ıt was found to be 0,73 for NRS-2002 and EDC. In our study, it was concluded that NUTRISCORE, a new screening test which was validated and tested in Turkish, can detect malnutrition risk of cancer patients diagnosed and treated in oncology outpatient clinics and can be used as screening test.
