Does treatment adherence predict children's emotion regulation in mentalization-based treatment for children (MBT-C) and a parenting and child social skills group (PSSG)?

Abstract

Uyum ve sonuç arasındaki ilişkiyi anlamak, bir tedavinin etkililiğini ölçmek için önemlidir. MBT-C, çocukların zihinselleştirme ve duygu düzenleme becerilerini iyileştirmeyi amaçlamaktadır. PSSG, çocuklarda sosyal becerileri ve duygusal düzenlemeyi geliştirmeyi amaçlamaktadır. Ancak zihinselleştirme temelli terapiler ve sosyal beceri grupları ile yapılan uyum çalışmaları sınırlıdır. Bu çalışma, uyumun çocukların duygu düzenlemesini yordayıp yordamadığını araştırmıştır. Her iki tedavide de modele daha güçlü uyumun, çocukların duygu düzenlemesini yordayacağı hipotezi öne sürülmüştür. İçe yönelim problemleri olan çocukların duygu düzenlemelerinde MBT-C ve PSSG'de modellere uyum daha güçlü olduğunda hafif düşüş görüldü. Komorbid problemleri olan çocukların duygu düzenlemeleri MBT-C'de uyum geliştikçe hafif düşerken, PSSG'de uyum geliştikçe belirgin bir değişiklik gözlenmedi. Bulgular, yeterli düzeyde uyumun önemli olduğunu göstermektedir. Dışa yönelim problemleri olan çocuklar, ihtiyaçlarının bireysel olarak ele alınacağı MBT-C'den fayda görebilirler. İçe yönelim problemleri olan çocukların daha esnek süreçlere ihtiyacı olabilir. Komorbid problemleri olan çocuklar ana odağın güven kurmak olduğu terapilerden daha fazla yararlanabilirler.Understanding the relationship between adherence and outcome is important to assess a treatment's effectiveness. MBT-C aims enhancing children's mentalization and emotion regulation abilities. PSSG aim improving social skills and emotion regulation in children. However, adherence studies on mentalization-based therapies and social skills groups are limited. This study investigated whether adherence predicted children's emotion regulation. It was hypothesized that stronger adherence to the model in either treatment will predict children's emotion regulation. Another aim was to explore via visual inspections how adherence affected emotion regulation of children with different problem types (internalizing, externalizing, comorbid) in two treatments. The sample consisted of 189 children randomly assigned to receive either MBT-C or PSSG, and received therapies simultaneously in parallel group design. 457 sessions were coded with MBT-C Adherence Scale and PSSG Adherence Scale. Emotion Regulation Checklist was collected regularly from parents regarding their children. Multilevel modeling analyses showed that adherence did not predict children's emotion regulation. Visual analyses revealed that emotion regulation of children with externalizing problems showed an increase as adherence improved in MBT-C, and a decrease as adherence improved in PSSG. There was a slight decrease in emotion regulation of children with internalizing problems in MBT-C and PSSG when there was stronger adherence to models. Emotion regulation of children with comorbid problems slightly decreased as adherence improved in MBT-C, while no considerable change was observed as adherence to PSSG improved. Findings suggest that good enough level of adherence is essential. Children with externalizing problems may benefit from MBT-C, where their needs could be addressed individually. Children with internalizing problems may need more flexible processes. Children with comorbid problems may benefit more from therapies where main focus is building trust.
