| dc.description.abstract | Özet: Türkiye’nin sağlık sistemi 2003 yılında uygulanmaya başlanan Sağlıkta Dönüşüm Programı ile köklü bir dönüşüm sürecine girmiştir. Sağlıkta Dönüşüm Programı öncesi ve sonrası Türkiye sağlık sisteminde kamu sağlık harcamalarının seyrini çıkarmak, gerek Türkiye’de sağlık sisteminin dönüşümü üzerine akademik tartışmayı ilerletebilmek, gerekse Türkiye için farklı sağlık politikası alternatifl erini tartışmaya açmak açısından önem arz etmektedir. Bu makale Sağlıkta Dönüşüm Programı öncesi ve sonrasını kapsayacak biçimde Türkiye’de kamu sağlık harcamalarının tarihsel seyrini çıkarmakta ve bu seyri sağlık politikalarındaki değişikliklerle birlikte değerlendirmektedir. Makalede 2006 yılından sonrası için kolaylıkla bulunabilen ayrıştırılmış kamu sağlık harcamaları verisi kütüphane taraması ve yazışmalar yoluyla elde edilen verilerle birleştirilerek 1988 yılına kadar geriye götürülmektedir. Bu verilere dayanarak makalede yıllık kamu sağlık harcamalarının gayrisafi yurt içi hasıla içindeki oranının reform öncesi dönemden itibaren artış eğiliminde olduğu ve reformun bu artış eğilimini tersine çevirmediği iddia edilmektedir. Kamunun sağlık harcamalarındaki bu artış büyük oranda Sosyal Güvenlik Kurumu cari sağlık harcamalarından ve özellikle de tedavi harcamalarından kaynaklanmaktadır. | en_US |
| dc.description.abstract | Abstract: Turkey’s health system has been undergoing a radical transformation process after the implementation of Health Transformation Program since 2003. Describing the historical trajectory of public health expenditures is important not only to advance the debate on transformation of Turkey’s health care system but also to open the debate for other health care policy alternatives for Turkey. This article provides data on the historical trajectory of public health expenditures that covers the timeframe before and after the reform and analyzes this trajectory against the background of health care policy changes. Using desk research and personal correspondance of authors with state off icials, the article compiles annual public health expenditures data, which are publicly available after 2006, back to 1988. Based on this data, the article suggests that the share of annual public health expenditures in gross domestic product has been in an upward trend before the reform and the reform did not reverse this trend. This rise in annual public health expenditures originates mainly from the increase in current health expenditures of the Social Security Institution and especially from expenditures on treatment services. | en_US |
