Enactment in psychodynamic psychotherapy: A quantitative case study on ruptures and repairs

Özet
Bu keşif niteliğindeki nicel vaka çalışması, psikoterapi araştırmalarında sıklıkla terapötik ittifak kırılmaları olarak anılan enactment (sahneleme) kavramını, terapötik bir ikilinin psikodinamik psikoterapi süreci üzerinden incelemektedir. Bu çalışmanın amacı, psikodinamik psikoterapide sahneleme kavramını hasta ve terapist kırılmaları, bunların çözülümü ve terapötik ittifak gibi gösterge yapılar aracılığıyla nicel olarak incelemektir. Bunu sağlamak için değişim trendleri, otokorelasyon ve çapraz korelasyon analizleri değerlendirilerek, değişkenler arasındaki zaman bağımlı ve birbirine bağımlı değişimler anlaşılmak istenmiştir. Çalışmada, bu yapıların sahneleme kavramının tanımlayıcı veya betimleyici unsurları olarak hizmet edip edemeyeceğini belirlemek için vektör otoregresif (VAR) analizi kullanılmıştır. Rupture Resolution Rating System (3RS): Version 2022 kullanılarak gerçekleştirilen seans bazlı gözlemsel kodlama, diyadik etkileşimlerin incelenmesine olanak sağlamıştır. Dokuz psikodinamik yönelimli eğitimli kodlayıcı, ilgili kodlama el kitabını kullanarak bu değişkenleri gözlemlemiş ve puanlamışlardır. Sonuçlar, hasta ve terapistin kırılma ve onarım değerleri arasında bir senkronizasyon olduğunu göstermiştir. Kırılma tezahürlerinin yoğunluğu ve netliği hasta ve terapist arasında değişkenlik gösterebilse de kırılmalar genellikle eş zamanlı olarak gerçekleşme eğilimindedir. Ayrıca, içerik analizi, sahnelemeyi karakterize eden tamamlayıcı roller ile kırılmalar arasındaki birbirine karşılık gelmeyi göstermiş; yüksek derecede kırılma olan seanslarda utanç ve suçluluk duygularının baskın olduğunu ortaya koymuştur. Granger nedensellik testleri neticesinde, terapötik ittifak ile terapist kaynaklı kırılma değerlerinin karşılıklı olarak birbirini öngördüğü tespit edilmiştir. Etki-tepki analizi sonuçları, sistemin içsel dengeli yapıda olduğunu işaret etmektedir. Çalışma sonuçları, 3RS 2022 versiyonunun sahneleme kavramını geçerli bir şekilde ölçebildiğini göstermektedir.
This exploratory, quantitative case study investigates the concept of enactment, often referred to as alliance ruptures in psychotherapy research, through the psychodynamic psychotherapy process of a therapeutic dyad. The aim of this study was to quantitatively explore the concept of enactment in psychodynamic psychotherapy by examining it via indicator constructs, including patient and therapist ruptures, their resolution, and the therapeutic alliance. To achieve this, trends of change, auto-correlations, and cross-correlations were analyzed to understand the time-dependent and interdependent changes among the variables. The study employed vector autoregressive (VAR) analysis to determine whether these constructs could serve as definitive or descriptive elements of the enactment concept. Observational coding, conducted on a session-by-session basis using the Rupture Resolution Rating System (3RS): Version 2022 allowed for the examination of dyadic interactions. Nine psychodynamic-oriented trained raters utilized a relevant coding manual to observe and rate these variables. The results revealed a synchronization between the patient's and therapist's ratings of rupture and repair. Although the intensity and clarity of rupture manifestations may vary between the patient and the therapist, the ruptures tend to co- occur. Additionally, content analysis showed a correspondence between ruptures and the complementary roles that characterize enactment, with shame and guilt being the dominant affects in sessions with a high degree of rupture. Granger causality tests indicated that the therapeutic alliance and therapist's rupture ratings bidirectionally predicted each other, and impulse-response analysis showed that the system is internally balanced. The study results demonstrate that the 2022 version of the 3RS can validly measure the relational concept of enactment.
