Diyabetli hastalarda Hipoglisemi Tutum ve Davranış Ölçegi'nin (HABS) Türkçe geçerlilik ve güvenirlilik çalışmasının yapılması ve beslenme alışkanlıkları ile ilişkisi

The Turkish validity and Reliability Study of Hypoglycemic Attitudes and Behavior Scale (HABS) in diabetic patients and relationship with nutritional habits Hypoglycemia, the main limiting factor in glycemic management and maintenance of diabetes; can cause serious anx.iety and fear in diabetic patients with disturbing symptoms and known negative consequences. This may weaken the selfmanagement of people with diabetes and, as a result, promote an avoidance behavior, creating a worse or unbalanced cycle of glycemic control. Given that diabetes management is based primarily on patients 'attitudes and behaviors towards their own health, and in particular, on their treatment, it may be necessary to correct the patients' misconceptions and beliefs about hypoglycemia in correcting their behavior. This research; In order to test the validity and reliability of the Turkish version of the Hypoglycemia Attitude and Behavior Scale (HABS) in T2DM patients and to evaluate the relationship between these attitudes and behaviors and nutritional habits, a methodological and descriptive type was planned. The study was conducted between June 2018 and December 2018 in Istanbul University, Istanbul Faculty of Medicine, Department of Internal Medicine, Department of Endocrinology and Metabolism Diseases Diabetes Clinic and was included 181 Type 2 diabetic patients. The mean duration of diabetes was 15.67 ± 7.14 and most (66%) were female Type 2 diabetic individuals. In the study as data collection tools; The Questionnaire for patient information form which includes sociodemographic, clinic characteristics, nutritional habit ete., 24-hour Food Consumption Record, Hypoglycemia Fear Scale (HFS), The World Health Organisation-Five Well-Being Index (WH0-5) and Hypoglycemic Attitude and Behavior Scale (HABS) were used. The language validity of the Turkish form of the scale was provided by the translation-back translation method. In addition, expert opinion was consulted for scale content validity and total Content Validity Index (CVI) was found to be 0,86. In order to determine the construct validity of the Turkish version of the scale, exploratory and confirmatory factor analyses were applied and 3 sub-dimensions (anxiety, avoidance and confidence) were obtained in accordance with the original scale. In order to evaluate the reliability of the Turkish version of the scale; internal consistency coefficient, item analysis, testretest and parallel form methods were used. The Cronbach's alpha value of the scale was found to be 0,818 (Confidence), 0,677 (Anxiety), 0,748 (Avoidance) for 3 subdimensions, respectively. In item analysis results, all item-scale total correlations for each sub-dimension were obtained at a significant level. In order to determine the invariance ofthe Turkish version ofthe scale against time, the scale was applied to 67 patients at four weeks intervals and it was found that there were positive, high and statistically significant correlations between the scale sub-dimensions of the two measurements (p <0.001). In addition, parallel forms reliability of the method of the original scale is in Turkey on the basis of work is improved and reliability made Hypoglycemia for Fear Scale (HFS) were used and between sub-dimensions on the scale of positive, the correlation was found in medium and high levels (p <0.001). Patients' hypoglycemia attitudes and behaviors; gender, marital status, presence of diabetes-related disease, insulin use, hypoglycemia event, HbAlc values, carbohydrate intake were found to vary. As a result; HABS is a valid and reliable scale that can be used to evaluate hypoglycemia attitudes and behaviors of patients with T2DM. Particularly, it is thought that the confidence subscale, which is one of the sub-dimensions of the HABS scale, has a positive correlation with the glycemic control, and that low levels of hypoglycemia confidence may be useful in preventing T2DM patients from developing a fear of hypoglycemia.
