Abstract

Faz açısı, beslenme durumunun klinik bir belirteci ve metabolik durumun seyrinde yararlı olan bir belirteçtir. Retrospektif yapılan bu çalışmada bariatrik veya metabolik cerrahi uygulanan en az II. derece obezlerin (BKİ?35 kg/m2) preoperatif ve postoperatif Biyoelektrik İmpedans Analizi (BİA) yöntemiyle ölçülen faz açısı değerleri ile biyokimyasal parametre ve antropometrik ölçümleri arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Ocak 202–Nisan 2023 tarihleri arasında cerrahiye başvuran, endikasyonları sağlayan, sleeve gastrektomi (SG) (n=6) ve transit bipartisyon (TB) (n=114) ameliyatları uygulanan hastalardan postoperatif 1., 3. ve 6. ay kontrollerine düzenli gelen diyabetik (n=81) ve non-diyabetik (n=39) olmak üzere toplamda 120 obez hasta çalışmaya dahil edilmiştir. Hastaların antropometrik ölçümleri ve faz açısı değerleri SECA MCA 515 marka tartı ile ölçülmüştür. Verilerin değerlendirilmesinde kullanılan tanımlayıcı istatistikler, Pearson Korelasyon Testi, ANOVA ve bağımsız t testleridir. Çalışma sonucuna göre faz açısının ortalaması ve standart sapmaları preoperatif dönemde 5,43?0,643 olan hastaların postoperatif 6. ayda 4,81?0,66'ya düştüğü saptanmıştır. Preoperatif ve postoperatif süreçlerdeki vücut kompozisyonu bileşenleri, faz açısını etkileyen temel faktörler olarak bulunmuştur. Bu bileşenler BKİ, yağ kütlesi, yağ yüzdesi, kas kütlesi ve TBW değerleridir. Faz açısı değeri ile beden kütle indeksi (BKİ), yağ kütlesi ve yağ yüzdesi arasında negatif ve anlamlı (p<0,05) ilişki bulunurken; kas kütlesi, yağsız vücut kütlesi (FFM) ve toplam vücut suyu (TBW) ile pozitif ve anlamlı (p>0,05) ilişki bulunmuştur. Beslenme ile ilişkili biyokimyasal verilerden ise preoperatif süreçte faz açısıyla D vitamini ve ferritin ile arasında pozitif ve anlamlı (p<0,05) ilişki saptanırken; preoperatif ve postoperatif bütün süreçlerde parathormon ile negatif ve anlamlı (p<0,05) ilişki saptanmıştır. Faz açısıyla hemoglobin arasında da preoperatif ve postoperatif tüm süreçlerde pozitif ve anlamlı (p<0,01) ilişki bulunmuştur. Cinsiyet faktörü vücut kompozisyonu farklılığına bağlı değerlendirilebilir. Yine preoperatif ve postoperatif süreçte beslenmeyle ilişkili parametrelerden hemoglobin ve parathormonun faz açısını etkilediği gözlemlenmiştir.Phase angle is a clinical marker of nutritional status and a useful marker of metabolic status. In this retrospective study, we aimed to evaluate the relationship between preoperative and postoperative phase angle values measured by Bioelectrical Impedance Analysis (BIA) method and biochemical parameters and anthropometric measurements of at least grade II obese patients (BMI?35 kg/m2) who underwent bariatric or metabolic surgery. A total of 120 obese patients, including diabetic (n=81) and non-diabetic (n=39) patients who were admitted to surgery between January 2021 and April 2023, who met the indications and underwent sleeve gastrectomy (SG) (n=6) and transit bipartition (TB) (n=114) surgeries, and who regularly attended the postoperative 1st, 3rd and 6th month controls were included in the study. Anthropometric measurements and phase angle values of the patients were measured with SECA MCA 515 weighing scale. Descriptive statistics, Pearson Correlation Test, ANOVA and independent t tests were used to evaluate the data. According to the results of the study, the mean and standard deviation of the phase angle decreased from 5,43?0,64 in the preoperative period to 4,81?0,66 in the 6th postoperative month. Body composition components in preoperative and postoperative periods were found to be the main factors affecting the phase angle. These components are BMI, fat mass, fat percentage, muscle mass and TBW values. A negative and significant (p<0,05) correlation was found between phase angle value and body mass index (BMI), fat mass and fat percentage, while a positive and significant (p>0,05) correlation was found with muscle mass, lean body mass (FFM) and total body water (TBW). Among the biochemical data related to nutrition, a positive and significant (p<0,05) relationship was found between vitamin D and ferritin with phase angle in the preoperative period, while a negative and significant (p<0,05) relationship was found with parathormone in all preoperative and postoperative periods. There was also a positive and significant (p<0,01) correlation between phase angle and hemoglobin in all preoperative and postoperative periods. The gender factor can be evaluated due to the difference in body composition. It was also observed that hemoglobin and parathormone, which are nutrition-related parameters, affected the phase angle in preoperative and postoperative periods.
