Practising elsewhere: Reasons, expectations and experiences of Turkish physicians in Germany

2013 yılından itibaren Türkiye'deki hekimlerin yurtdışına göç etme eğilimlerinde belirgin ve hızlanan bir artış görülmektedir. Bu nedenle, bu yılı tarihsel bir referans noktası olarak ele alan bu çalışma, Lee'nin itme-çekme teorisi çerçevesinde Türkiye'den Almanya'ya yönelik hekim göçünün altında yatan faktörleri belirlemeyi ve hekimlerin Almanya'daki deneyimlerini, entegrasyon süreçlerini, bireysel motivasyonlarını ve geleceğe yönelik beklentilerini derinlemesine incelemeyi amaçlamaktadır.Bu nitel araştırma, tıp eğitimini Türkiye'de tamamlamış ve görüşme sırasında Almanya'da aktif olarak mesleğini icra eden 14 hekimle gerçekleştirilen yarı yapılandırılmış derinlemesine görüşmelere dayanmaktadır. Çalışma, hekimlerin karar verme süreçlerine ilişkin önemli içgörüler sunmakta ve göçü şekillendiren faktörlerin daha kapsamlı bir şekilde anlaşılmasına katkı sağlamaktadır. Sosyo-kültürel, ekonomik, siyasi ve bireysel boyutların yanı sıra, araştırma meslekleri ve göç deneyimleriyle ilişkili yapısal ve kurumsal faktörlerin etkilerini de incelemektedir. Bulgular, hekim göçünün yalnızca profesyonel nedenlerle açıklanamayacağını; aksine Türkiye'nin sosyo-politik bağlamı, cinsiyet ve cinsel yönelim, aile ve partner etkisi, bireysel motivasyonlar ve gelecek kaygısı gibi unsurlarla şekillenen karmaşık ve çok katmanlı bir yapıya sahip olduğunu ortaya koymaktadır. Halk sağlığı ve stratejik planlama açısından, hekim göçünün dinamiklerini anlamak ülkeler için kritik öneme sahiptir. Bu doğrultuda, çalışma her iki ülkedeki politika yapıcılar ve diğer paydaşlar için değerli bir referans niteliğindedir.

Since 2013, there has been a marked and accelerating increase in the tendency of physicians in Türkiye to migrate abroad. Therefore, taking this year as a historical reference point, this study aims to identify the factors underlying physician migration from Türkiye to Germany within the framework of Lee's push–pull theory, and to examine in depth their experiences in Germany, their integration processes, their individual motivations, and their expectations for the future. This qualitative research is based on semi-structured, in-depth interviews conducted with 14 physicians who completed their medical education in Türkiye and were actively practising in Germany at the time of the interviews. The study provides significant insights into physicians' decision-making processes and contributes to a more comprehensive understanding of the factors shaping migration. In addition to socio-cultural, economic, political, and individual dimensions, the research also explores the influence of structural and institutional factors related to their profession and migration experiences. The findings reveal that physician migration is not driven solely by professional considerations; rather, it reflects a complex and multilayered structure shaped by Türkiye's socio-political context, gender and sexual orientation, the influence of family and partners, individual motivations, and concerns about the future. From a public health and strategic planning perspective, understanding the dynamics of physician migration is crucial for countries. Accordingly, this study offers a valuable reference for policymakers and other stakeholders in both nations.
